System of Care (SOC) is a community-based program funded through a contract with the Illinois Department of Children and Family Services (DCFS). The program is designed to plan, organize, staff, and administer an array of critical, intensive therapeutic interventions and/or facilitation services to emotionally and behaviorally disturbed clients for whom DCFS is legally responsible in order to stabilize their traditional or home of relative foster care placement. The SOC provider works in conjunction with the foster care agency to best meet clients' needs either through additional services, or through existing contracts.
The Center for Family Services®' SOC program serves eligible clients who reside in LANS 47 and 49 (i.e., Kane, Kendall, Will, and Grundy counties). If you are currently working with a child who you feel would benefit from SOC services, you may contact Shanna Barnes at (630) 632-7394 or (630) 990-7319 for clients in Kane, Kendall, Will or Grundy counties to complete the referral process.
Referrals can be made on an emergency basis, 24 hours a day, by contacting the CARES line at (800) 345-9049, if the risk for placement disruption is immediate.
